Dry eye is something that affects all of us sooner or later in life. Most optometrists and ophthalmologists will treat dry eyes with topical artificial tears and prescription medications or punctal plugs. Many patients will get symptomatic relief with these traditional approaches for the treatment of dry eyes. However, very few doctors will discuss dry eye nutrition with their patients. I have seen the power of proper nutrition for dye eyes in clinical practice for almost 20 years. One of the most frequent questions I am asked on my live talks show “Ask The Doctor” is, “What are good supplements for dry eye?” or, “Are there dry eye vitamins that really work?” The answer is yes, there are many dry eye supplements and vitamins for dry eyes on the market today. However, “Do dry eye vitamins actually work?” is the big question. Dry eye nutrition is one of the key discussions I have with my patients at Lange Eye Care. Most patients I see with a dry eye condition have never been educated on the power of nutrition for dry eye treatment. People in today’s society want to become proactive in their overall health and are looking for a healthy option to medications. Natural dry eye treatments are becoming very popular because they work! A consumer must be very careful when shopping for dry eye vitamins over the counter at the local drug store or warehouse club because most of these over the counter supplements for dry eyes are filled with dyes, trans fats and synthetic derivatives that may actually cause harm with long term usage. The typical vitamins for dry eye that are available commercially are also usually relatively weak in concentration and may not have much therapeutic benefit.

Dry eye nutrition consists of a dry eye diet as well as dry eye vitamins. Both of these work together in the natural treatment of dry eyes. When the traditional approaches for dry eye treatment are combined with the natural dry eye treatment it is even more effective. First and foremost I will make sure the patient adjusts their diet to one similar to the “paleo diet” and I will make sure they are drinking plenty of the correct fluids.

The next step in dry eye nutrition is to get the patient on a good triglyceride form omega-3 fish oil like Fortifeye Super Omega. The size of the patient and the severity of the dry eye condition will determine how many Fortifeye Super Omega gel caps I recommend. For anyone over 250 lbs I recommend 3-4 capsules or if they have a significant dry eye. For everyone else I recommend two Fortifeye Super Omegas a day. Fortifeye Super Omega has an EPA of 860 and a DHA of 580 in two gel caps. This omega 3 fish oil has been tested in an omega index study and is able to move the omega index on an average over 9.1%. This is a true therapeutic dosage in just two gel caps per day on most individuals. The average American has an omega index of only 4%. This is the ratio of omega-3 to omega-6 in our blood. We are finding our patients with an omega index above 9% have fewer dry eye symptoms than the ones below this level.

Fortifeye Super Omega is being called dry eye fish oil by our patients that have reduced their dry eye symptoms while taking this form of omega-3 fish oil. Fortifeye Super Omega was developed as a chewable for those that can’t swallow it. It has a good lemon flavor and will not cause any burp back or after taste. This omega-3 fish oil is very similar in concentration to prescription fish oil that costs ten times the amount. I also recommend eating more sardines and wild Alaskan salmon as part of the dry eye diet.

Introducing astaxanthin – the super carotenoid

Many people suffering with dry eyes will benefit significantly from the addition of the Fortifeye Super Omega in a 2-4 per day dosage but some may need the addition of one more nutrient called astaxanthin. I have been using 6 mg of astaxanthin in patients that have dry tired eye symptoms with some very positive results.

Astaxanthin has many potential health benefits for the visual system and the entire body. Astaxanthin is a super carotenoid found in specific algae. This carotenoid at 6mg per day dosage can help to relieve tired fatigued eyes. Clinical studies have proven that 6mg of astaxanthin actually improves the focusing ability of the human eye.

Ever since I have seen the studies on astaxanthin I began using it clinically to help with eye fatigue. I have found that dry eyes and tired eyes go hand in hand and the addition of 6mg of astaxanthin (Fortifeye FOCUS) to 2 gel caps of Fortifeye Super Omega have been extremely helpful when used together in dry eye nutrition. Now routinely I use the combination of Fortifeye Super Omega and Fortifeye FOCUS as my supplements for dry eye. Many of our patients are omega-3 deficient and have an excess of omega-6. Simply by having an omega index blood study done the typical patient will see if they are deficient or not in omega-3. Once you get your omega index levels upward of 8% you are not only in a heart healthy zone but you will also be in the zone that may help decrease your dry eye symptoms.

Take a minimum of 860 mg EPA and 580 DHA from a good triglyceride form omega-3 fish oil. Take 6mg of astaxanthin daily.

I also recommend taking a good multivitamin (Fortifeye Complete One-A-Day) with no beta carotene or vitamin A to ensure that there are no nutritional deficiencies that could interfere with the entire metabolic pathway. Try to see an eye care provider that is in tune with nutrition routinely as well. More information: Dr. Michael P. Lange is a Board Certified Optometric Physician and a Certified Nutrition Specialist who started Lange Eye Care and Associates in Ocala, Florida, in March 1993. Dry eyes can be improved through proper nutrition. The vast majority of dry eye patients will notice a significant improvement in dry eye symptoms once the correct dietary and lifestyle modifications are implemented. Dry eye is a nutritionally responsive condition and this paper will discuss how proper diet and supplementation may improve subjective and objective signs and symptoms of dry eyes.

Most of our patients, whether fat or thin, are nutritionally compromised. Very rarely will you meet a patient that is of ideal body weight, medication free and on a truly healthful diet. As a clinician you will know at the first glance of your patient and after a brief discussion how critical a dramatic adjustment in their diet is going to be to their overall health and their dry eyes. Proper nutrition is one of the most important single factors in maintaining the health of our patients. Most people in today’s society are not getting the required vitamins, minerals and essential fatty acids from the foods they are eating. We have become a society living ‘life on the go’ that is fueled by fast food and junk food. Our typical diet is caloric dense and nutrient deficient. Americans consume a diet that is unbalanced—with too many omega-6 fatty acids and not enough omega-3 fatty acids—leading to increased inflammation and disease in the body. This is associated with increased levels of cytokines, proteins that trigger inflammation. Inflammation is one of the root causes of dry eye syndrome (keratoconjunctivitis sicca) and many other systemic diseases. Our foods are processed, sugar- and salt-laden, filled with trans fats, hydrogenated oils, high fructose corn syrup, synthetic dyes, chemicals and artificial sweeteners. These poor nutritional habits combined with a sedimentary lifestyle are why the health of the American people (our patients) is becoming compromised. America is home to the most obese people in the world. According to the CDC (Center for Disease Control and Prevention) obesity in adults has increased by 60% within the past twenty years. A staggering 70% of American adults are overweight or obese and obesity related deaths have accelerated to more than 300,000 a year, second only to tobacco related deaths.

Most nutritional experts recommend 10-13 servings of organically grown fruits and vegetables a day. Americans are falling short of this amount and are becoming more and more nutritionally compromised. 70% of adults are not even consuming five portions of fruits and vegetables a day. The population that consumes the least amount of vegetables and fruits are more predisposed in developing disease and illness. These large and growing groups of people are nutritionally compromised and more prone to develop free radical damage in their bodies and eyes. These are the patients who are more prone to have dry eyes, cataracts and retinal disease. I have seen in clinical practice for close to twenty years the positive effects dietary modifications and lifestyle changes can have directly on ocular health and total body health.

I have finally come to the realization that patients who will follow a combination of the Mediterranean, paleo or hunter gatherer type of diet, eating more like our ancestors, along with regular exercise and proper sleep benefit significantly. This is a diet that consists of organically grown fruits, vegetables, seeds, nuts, whole grains, proteins and mainly water and a few other beverages. I recommend patients avoiding water that is in plastic bottles and drinking water that is filtered or in glass bottles due to the potential for bisphenols leaching into the water from the plastic. Bisphenols cause endocrine disruption which will compound a dry eye problem. Bisphenols are in plastic bottles as well as in canned foods. F, D and C red yellow and blue dye can also cause endocrine disruption and so should be avoided. Dyes are in just about everything we consume these days, so teach your patients to be proactive in their health and read labels. I recommend patients try to consume half their body weight in ounces of good quality water daily. I also advise patients to try to substitute their morning coffee with organic green tea and lemon. The antioxidant and anti inflammatory effects of green tea and lemon have significant health benefits and many of my patients will notice symptomatic relief of their dry eyes from this one simple step. I advise my patients to try to eat 10-13 servings of fruits and vegetables on a daily basis. I recommend “the dirty dozen” fruits and vegetables be only organically grown due to their high amounts of pesticide found inside this specific group. Most of us have an accumulated build up of pesticide exposure in our bodies due to numerous years of exposure. This “chemical burden” leads to a variety of serious health issues that may potentiate a dry eye condition. The protein in this particular type of diet should always be free range organically raised or wild, not farm raised! Try to get your patients to eat fish at least three times per week to improve the omega three to omega 6 ratio in their blood. The fish that is highest in omega three and lowest in potential contaminants are sardines, anchovies, herring, mackerel, wild rainbow trout and wild Alaskan salmon. Some of your patients will follow your directions precisely; however the majority is going to wonder off of these specific recommendations. This is why supplementation with antioxidants and omega-3 fatty acids is so important.

Blood chemistry testing

How do you know the specific nutritional status of your patients without testing them? The only accurate way to test for these deficiencies is through blood testing. We will routinely do three special blood tests to gain valuable insight into the nutritional health of our patients. A SpectraCell intracellular blood test, vitamin D 25 OH and an omega-3 index test will help determine what your patient’s nutritional status is and if they have nutritionally modifiable risk factors for ocular conditions like dry eyes. You will quickly see that almost everyone you test is deficient in antioxidants and omega three. Many of these patients will also have elevated CRP, homocystein, triglycerides, cholesterol and arachidonic acid. The majority of these blood chemistries can be positively impacted through proper nutrition. The poor results on these tests can help motivate your patients to making these crucial dietary and lifestyle modifications that will positively impact their overall health. After reviewing the results of these tests with your patients they will finally understand the benefit of proper supplementation. Realistically, due to these tests not being covered by Medicare or insurance, not everyone will be able to have these tests done because of the cost. Let’s now review the specific protocol that is working extremely well with the dry eye population.

Omega-3 fish oil

The majority of large clinical studies that have documented the positive health benefits of omega-3 fatty acids have utilized fish or fish oil consumption. There is a multitude of positive studies supporting the beneficial health aspects of a higher consumption of omega-3 from fish oil or fish. The medical community has readily embraced these positive health benefits and is prescribing and recommending fish oil supplements to their patients. The increased public awareness regarding omega 3 fish oil has created an omega-3 explosion. Consumers that are in tune with prevention and wellness are becoming proactive in their health and practicing prevention by consuming more fish or purchasing fish oil supplements from the drug store, health food store or through the Internet. However, one must be careful with consuming too much fish because of the potential contaminants that are in particular types of fish. With all of the contamination in the oceans it is becoming more and more difficult to recommend consuming large amounts of fish. The consumer or patient must also be aware that all fish oil is not created equal! The health care professional should be educating their patients on the benefits of proper omega-3 supplementation.

Ethyl ester based fish oil eats Styrofoam!

Omega-3 fish oil has two readily available forms: ethyl ester fish oil and triglyceride form fish oil. I recommend purchasing the triglyceride form fish oil over the ethyl ester version of omega-3 fish oil for a variety of reasons. The ethyl ester form of fish oil is highly unstable and therefore it can oxidize and become rancid quicker than the triglyceride form of fish oil. Many of the ethyl ester fish oils may become rancid before you purchase them. This is one of the reasons many people get a nasty aftertaste or have problems burping up an unpleasant fish taste from the ethyl ester form of fish oil. Studies also indicate that the triglyceride form omega-3 fish oil absorbs much faster and more efficiently therefore increasing bio availability. Most fish oil that is being sold OTC or as a prescription in the United States is the ethyl ester version because it is significantly less expensive to manufacture. The final phase in the purification of a triglyceride form of fish oil requires the ethyl ester molecule to be cleaved off and replaced with a triglyceride molecule, thus making it a more natural omega-3 fish oil than the ethyl ester version. The triglyceride form of omega-3 fish oils are extremely pure and concentrated making them the ideal choice for your patients. If you do not know if your fish oil is an ethyl ester version or triglyceride version you should call the customer service department on the back of the bottle. You can also squeeze the contents of your fish oil into a Styrofoam cup and watch for the chemical reaction. If the cup begins to dissolve after 10 to 20 minutes, then you have an ethyl ester form. However, some fish oils with very low concentrations of ethyl ester fish oil may not dissolve the cup. When shopping for the best triglyceride form fish oil, look for high levels of EPA and DHA. I recommend finding an omega-3 fish oil supplement that has an EPA between 400-450 per gel cap and a DHA around 250-300 per gel cap. These are the levels I have studied using the latest in omega-3 blood testing and have found that the consumption of two gel caps a day of this concentration can increase your omega index to a desirable level over 8%. This omega-3 index measures the concentration of EPA and DHA as a percent of total essential fatty acids in the red blood cell membranes. Recent research is proving that the desirable level for cardio protection is an omega index over 8%. This research claims that levels greater than 8% correlate with a 90% reduction in the potential for a fatal heart attack. The vast majority of Americans have an omega index closer to 4%.

My research team and I have tested a variety of omega-3 oils from flax, krill and ethyl ester fish oils and have not found nearly the improvements in the omega index with these other types of omega-3 fatty acids. So all omega-3 supplements are not created equal! Most of the inferior brands of fish oil and krill oil you can purchase OTC have no therapeutic value and won’t move the omega index very much. Many of the OTC fish oils are also filled with higher than the acceptable amounts of contaminants like PCB. Is this something you want to recommend to your patients? After years of study, a lot of trial and error and a one month research trip to Norway, I developed a highly purified triglyceride omega-3 fish oil concentrate with an outstanding 1600mg of omega-3 (EPA 860mg, DHA 580mg) in just two gel caps! The teams at Fortifeye Vitamins and the Omega 3 Test have been actively studying the influence of two daily capsules of this concentrated Super Omega on blood levels of omega-3 and the results have been astounding! After just thirty days, mean blood levels of omega-3 have increased from 4.2% to 9.1% with some patients as high as 11%! We have positive results using these levels of omega-3 fish oil with our MGD patients, those with recurrent hordeolums, conjunctivitis, chronic low grade uveitis, blepharitis and we are even seeing positive impact on our diabetic maculopathies and macular degeneration patients. Based on the current world wide research available, combined with the ongoing research being done at Fortifeye Vitamins, I feel the primary eye care provider should be making omega-3 recommendations to all of their patients. EPA (eicosapentaenoic acid) and DHA (docosahexaenoic acid) are the two principal EFAs (essential fatty acids) found in fish known as omega-3 fatty acids. The current research suggests the anti-inflammatory effects of fish oil may help with dry eye syndrome. EPA generates the anti-inflammatory eicosanoids prostaglandin E3 and leukotriene B5. If the concentration of EPA is high enough the EPA blocks the delta-5 desaturase enzymatic step that stops the conversion of omega-6 EFAs to the inflammatory cascade of arachidonic acid (AA). This allows more conversion to anti-inflammatory prostaglandin E1 (PGE1). PGE1 then binds to EP2 and EP4 receptors to activate adenylate cyclase and increase cyclic AMP (adenosine monophosphate) which in turn stimulates a healthier tear production. DHA also decreases production of pro-inflammatory eicosanoids and helps to balance the body’s response to inflammation. Both EPA and DHA contribute to reducing inflammation in somewhat different but complementary mechanisms. The increase in omega-3 EFAs has also shown to improve dysfunction in the meibomian gland with a much thinner and clearer meibomian gland secretion, thus helping with meibomian gland dysfunction. The increased consumption of a triple molecularly distilled and concentrated triglyceride form omega-3 fish oil will help to get the balance between omega-6 and omega-3 in better harmony and allows for less inflammation in the body and eyes. Americans have too high a percentage of pro-inflammatory omega-6 in their diet. The addition of some good omega-3 fish oil is the start to a healthier body and visual system.

A relatively new nutrient used in supplements that comes from the micro alga haematococcus pluvialis, astaxanthin is proving to have multiple health benefits. The super carotenoid astaxanthin is the reason lobsters, shrimp, crabs and wild salmon have a reddish color. Astaxanthin is an extremely potent antioxidant and anti-inflammatory. These patients will also commonly report an improvement in many other areas: less joint pain, healthier skin, improved energy, improved muscle recovery, more endurance, improvement in focusing and many more. I have seen a synergy with triglyceride form fish oil while using astaxanthin in my dry eye population. If patients are not experiencing the relief they are looking for on omega-3 supplementation alone I will add 6 mg of astaxanthin and see the patient back in three months for follow up. I have been amazed at the positive feedback I have received from these patients. Not only are these patients improving subjectively but their objective signs of dry eye are also improving. I think astaxanthin has a place in eye care especially for the patients who are looking for a more natural approach to help with their dry tired eyes. Think about how many patients the combination of omega-3 and astaxanthin may help in your practice. Just about everyone these days is experiencing some form of dry and fatigued eye symptomology throughout the day. Patients are grateful when you help solve their complaints without recommending surgery or medication.

Gamma-linolenic acid

The moderate to marked dry eye patients may need one additional nutrient in conjunction with the omega-3 and astaxanthin to maximize the nutritional approach for dry eyes. Gamma-linolenic acid (GLA) is an essential fatty acid (EFA) in the omega-6 family that is found mainly in plant seed oils. The three most common plant seed oils where GLA are found are borage seed oil, black currant oil and evening primrose oil. GLA is broken down in the body to dihomo-gamma-linolenic acid (DGLA) and arachidonic acid (AA). DGLA competes with AA and prevents the negative inflammatory effects that AA would otherwise cause. Adequate amounts of magnesium, zinc and vitamins C, B3 and B6 help to promote the conversion of GLA to DGLA rather than AA. It is fairly rare that our patients will have deficiencies in these areas if eating the appropriate foods. However, many of our patients are not getting the appropriate nutrition in their diet and must supplement with a good whole body anti-oxidant. Running a SpectraCell nutritional blood panel may help find many surprising nutritional deficiencies your patients have. Researchers in Japan have shown that astaxanthin may suppress the cyclooxygenase enzyme and decrease the amount of pro-inflammatory PGE2 produced. Once the AA pathway is blocked the DGLA is free to convert to anti-inflammatory prostaglandin E1 (PGE1) and stimulation of tear production occurs. Recent studies suggest that oral GLA supplementation improved dry eye both clinically and subjectively in patients with dry eye syndrome. Studies also show that patients who take a combination of linoleic acid (LA) and gamma-linoleic acid (GLA) prior to and after refractive surgery may have better tear production and less dry eye complications. Studies utilizing GLA and omega -3 fish oil have shown an additive anti-inflammatory effect with no elevation in arachidonic acid. Due to the extremely high amounts of omega-6 in most diets I began studying different ratios of omega-3 and GLA and have decided to use a ratio of approximately 10 to 1 omega-3 to omega-6 (GLA) to maximize the anti-inflammatory effects and gain full advantage of both types of essential fatty acids. If the ratio of omega-6 (GLA) to omega-3 is too high in the omega-6 direction then the possibility of more inflammation related to the high omega-6 low omega-3 ratio exists. Currently for my more advanced dry eye cases I have been using 1600 mg of triglyceride form omega-3, 160 mg of GLA from black currant seed oil, 6mg of astaxanthin and a good whole body anti-oxidant with great results. These patients not only experience ocular health benefits but also notice many general health benefits as well. It is time that both optometrists and ophthalmologists start to utilize the power of proper nutrition to help their patients. It is a win-win for everyone involved.

More information: Dr. Michael P. Lange is a Board Certified Optometric Physician and a Certified Nutrition Specialist who started Lange Eye Care and Associates in Ocala, Florida, in March 1993. More information: Dr. Michael P. Lange is a Board Certified Optometric Physician and a Certified Nutrition Specialist who started Lange Eye Care and Associates in Ocala, Florida, in March 1993. Dr. Michael Lange, Ocala eye doctorMichael Lange, a native of New Orleans, Louisiana, received two undergraduate degrees in psychology and physiological optics before graduating from optometry school in Pennsylvania in 1992. While working in New Orleans, right out of school, gaining valuable experience at the Eye Surgery Center of Louisiana and at St. Charles Vision he did a demographic study of the country to determine where an optometrist would be most needed. His study determined that one of the fastest growing retired populations in the entire country in 1990 was a city in Marion County, Florida, called Ocala. Michael Lange, optometrist from New Orleans decided to leave his home town, family and friends and move to Ocala on March 15, 1993. This was a gamble that paid off for Michael Lange, Ocala eye doctor.

The Ocala eye doctor builds his reputation

Dr. Michael Lange moved to Ocala by himself and started Lange Eye Care and Associates in March, 1993. Dr. Lange states, “I was in the right place at the right time.” Dr. Lange fell in love with Ocala, Florida, and the surrounding areas. He said the landscape in Ocala, Florida, was not typical of Florida with its rolling hills and huge grand daddy oaks. The large grand daddy oak trees with the Spanish moss reminded him of the trees along St. Charles Avenue and Audubon and City Park in New Orleans. Dr. Lange was asked to host his own talk show called “Ask The Doctor” on a local AM radio station shortly after starting his first eye care center in Ocala. This talk show became quite popular and he began hosting this popular talk show on multiple radio stations in Ocala and Gainesville. The show focused on eye care and nutrition and was a live call-in show broadcast for one hour strategically placed during drive times in the morning, at noon and in the evening. Dr Lange’s eye care philosophy and smooth way of handling each caller on his radio show became a huge hit and sparked the beginning of his aggressive growth and expansion. Dr. Lange quickly became known as Michael Lange, the “Ocala Eye Doctor” that everyone wanted to see. Dr Lange’s reputation as the eye doctor that truly cares quickly spread and patients would drive from hundreds of miles around to see Dr. Michael Lange in Ocala, Florida. Dr Lange quickly expanded to Gainesville, Florida, and opened the second location of Lange Eye care in 1994. Dr. Lange quickly outgrew both of his locations in Ocala and Gainesville and in 1998 purchased a 15,000 square foot building on State Road 200 across from the Paddock Mall that became the headquarters for Lange Eye Care. He also moved his Gainesville office away from the mall and put it next to Target on Archer Road in a 7,000 square foot space. He then started to expand around his home office in Ocala, Florida, and opened offices in Williston, Inverness, Summerfield, West Ocala, The Villages and Clearwater.

The Lange Eye Institute

The Lange Eye InstituteThe Lange Eye Institute was built in the Villages in 2006 and is the flagship or crown jewel of the Lange Eye Care expansion. Dr. Michael Lange the Ocala eye doctor pulled from his New Orleans culture and decided to build a state-of-the-art eye care center that was modeled after an old plantation on the Mississippi river. The Lange Eye Institute is close to 30,000 square feet and is the largest eye care center of its type in the area. The Lange Eye institute in The Villages has specialty suites built for all aspects of eye care including pediatric, medical, surgical, nutritional and optical eye care. The Lange Eye Institute is home to many nutritional absorption studies developed by Dr. Michael Lange. Nutrition, Macular Pigments and AMD

The role of lutein and zeaxanthin supplements in nutrition, as discussed by leading optometrists, and their effects on macular degeneration and macular pigments. Taking lutein, zeaxanthin and dark green leafy vegetables such as spinach and kale can help with ocular nutrition. Caution is recommended for those with kidney stones or using blood thinners. Comments by Dr. Michael Lange OD CNS, certified nutrition specialist.

But just how much is needed and, how do we measure results?

It’s commonly understood: Levels of the macular pigments lutein and zeaxanthin correlate to agerelated macular degeneration risk (AMD). So, as a measure of prevention, should optometrists recommend dietary and supplemental intake of these important nutrients — carotenoids that protect against phototoxic blue light and near-ultraviolet radiation and which are found naturally in the macula?

And if so:

How much of each is needed?
Should the nutrients come from food, supplements or a combination of both?
How can you determine the success of a supplement program? Should you consider the use of devices that measure the macular pigment optical density (MPOD)?

Here, experts on ocular nutrition and MPOD-measurement devices weigh in on these, among other common questions.

How much is enough?

It’s difficult for healthcare professionals and patients to know just how much lutein and zeaxanthin should be consumed on a daily basis. Supplements with lutein and zeaxanthin vary in nutrient content and packaging. For instance, they often accompany vitamins and nutrients, such as vitamins A, C, and E and zinc. And because no recommended daily intake for these nutrients exists, many O.D.s aren’t sure how much to recommend for optimal macular health benefits.

“In general, normal lutein/zeaxanthin supplements average in the 15 mg to 20 mg (range) for lutein and 2 mg to 4 mg for zeaxanthin,” says Jeffrey Anshel, O.D., F.A.A.O., president of the Ocular Nutrition Society. “Some supplement companies add more, but most of the science points to about these amounts.”

He adds the caveat: “Since lutein is sequestered in fat tissue, different patients may require different amounts.”

There’s no exact number, explains Paul S. Bernstein, M.D., Ph.D., Mary Boesche professor of Ophthalmology and Visual Sciences at Moran Eye Center at the University of Utah School of Medicine in Salt Lake City. “Based on epidemiology and studies on risk of macular degeneration, it’s generally come up somewhere between 6 mg and 10 mg [of each] a day. Those are the reasonable numbers.”

Dr. Bernstein and others say they hope that tests of 10 mg of lutein and 2 mg of zeaxanthin in the second Age Related Eye Disease Study (AREDS2) study will provide a definitive answer.

Michael Lange, O.D., C.N.S. (certified nutrition specialist), who owns Lange Eye Care and Associates in Florida and also founded Fortifeye Vitamins, recommends dosages by age and health status. For most patients, he says he advises 6 mg to 10 mg of lutein and 1 mg to 2 mg of zeaxanthin daily. But for patients with early-to-moderate AMD, he says he recommends 10 mg to 20 mg of lutein and 2 mg to 4 mg of zeaxanthin daily. For moderate-to-advanced AMD, he says he recommends 20 mg to 30 mg of lutein and 4 mg to 6 mg of zeaxanthin.

But are lutein and zeaxanthin equally important?

The answer is unknown, says Elizabeth Johnson, Ph.D., with the Carotenoids and Health Laboratory of the Jean Mayer United States Department of Agriculture (USDA) Human Nutrition Research Center on Aging at Tufts University in Boston. She points to interesting data.

“Work that we conducted here shows definitively that of the zeaxanthin in the retina, about half of that is actually derived from lutein,” she says.

From food or supplements?

Recommended dosages of the nutrients can be absorbed from food or supplements, but some experts say it’s unlikely many patients will derive lutein and zeaxanthin from diet alone.

Kimberly Reed, O.D., F.A.A.O., associate professor at the College of Optometry at Nova Southeastern University in Fort Lauderdale, Fla., and a member of the Nutrition Ocular Society’s board of directors, says sufficient amounts of the nutrients would be difficult to amass from the typical Western diet.

“This is where patient education is essential,” Dr. Reed says.

A look at quantities of lutein in foods sheds light on how difficult levels experts recommend may be to reach, she explains. While dark leafy greens, like spinach and kale, abound with lutein at 20.4 mg and 23.7 mg per cooked cup, respectively, other foods considered good lutein sources contain far less. For example, corn and green beans — consumed more often than spinach and kale — contain just 1.5 mg and 0.9 mg per cooked cup, which presents a recommended-dosage shortfall if dark leafy vegetables aren’t also consumed, according to the Lutein Information Bureau (www.luteininfo.com) and the USDA’s Nutrient Database.

Dr. Johnson adds that patients who can’t eat green leafy vegetables because they’re on blood thinners or have a propensity toward kidney stones may benefit from lutein and zeaxanthin supplements.

Dr. Lange says he also believes in starting with diet and supplementing only if necessary.

“I will … tell my patients who aren’t on blood thinners and do not have kidney stones or gout to eat one-half-to-one-cup of dark green leafy vegetables a day,” he says.

But, Dr. Lange explains that even people with specific lutein and zeaxanthin restrictions usually don’t obtain sufficient amounts of lutein and zeaxanthin from their daily diet. This may be particularly true for the elderly, who may have limited access to shopping outlets, he says. They may need supplements, which Lange recommends “as an insurance policy for anyone with AMD.”

When should patients start?

At what age should patients start taking lutein and zeaxanthin supplementation, either through diet or supplements? Some experts suggest that people who have low MPOD should begin supplementation in their 20s. Our experts weren’t so sure.

“We know that interventions for macular degeneration are important when people are in the middle stages of macular degeneration,” Dr. Bernstein says.

When it comes to younger people with a family history of AMD, it’s more difficult to make recommendations, he adds.

“Patients have to realize the data are not as strong if they’re in their 20s and 30s,” he says. “I, in my practice as a macular degeneration specialist, counsel them most importantly to achieve a good, healthy diet: dark green, leafy vegetables and orange and yellow fruits and vegetables.”

Dr. Johnson adds that younger patients are most at risk for light damage and should know the importance lutein- and zeaxanthin-rich foods.

“The younger people are those who have the clear lens, and that makes the retina more vulnerable to light damage from being outdoors more often,” she says.

Supplements: How far should O.D.s go?

Should O.D.s recommend, or sell, lutein and zeaxanthin supplements to patients? Our experts say, “Yes,” with care and diligence.

“I make recommendations on supplements … and have them available as a convenience — just like I do glasses and contact lenses,” Dr. Anshel says. “There are several factors that the doctor must consider when evaluating a nutritional supplement, including knowing the science behind the ingredients and having a company that is third-party GMP (Good Manufacturing Practices) certified.”

The decision to recommend or sell supplements largely depends on the O.D.’s personal preference, Dr. Reed says, stressing responsibility.

“Many optometrists prefer to avoid the ‘retail’ end of nutrition, while others have seamlessly incorporated this into their already successful retail business,” she says.

She cautions that it is paramount that optometrists who begin nutritional counseling or retailing, or both, be properly educated on the topics and remain up-to-date on nutrition and the eye.

Dr. Lange recommends offering patients a whole-body supplement with enough lutein/zeaxanthin, along with various other vitamins, minerals, wholefood complexes and omega-3 essential fatty acids.

“Lutein and zeaxanthin are only two pieces of the puzzle,” Dr. Lange says, adding that optometrists should take nutrition courses and consider becoming a certified nutrition specialist (CNS) for confidence in helping patients with nutrition.

Measuring results

Of the methods used to measure MPOD, optometrists are likely most familiar with commercially available heterochromatic flicker photometry (HFP) devices. The authors of a recent review article in Clinical and Experimental Optometry (September 2010) write that due to concerns regarding the repeatability of test results, they recommend clinicians exercise caution when considering these instruments to monitor MPOD in patients who are taking lutein and zeaxanthin supplements.

Dr. Bernstein agrees with this conclusion, noting that HFP is a subjective test that is difficult for many people, even well-trained professionals, to use. He adds that often the operator or patient is not trained adequately in these devices.

“If you don’t have proper training, you’re going to get results that don’t mean very much,” he says.

So, should optometrists purchase and use the HFP devices to assess MPOD in patients? Dr. Bernstein answers “yes,” “with a healthy dose of caution.” He encourages optometrists to try the devices to determine reproducibility in their practices.

The vendors of the HFP measuring devices may also be able to lend assistance in this area as they offer training on the devices.

Dr. Anshel recommends taking a scientific approach to the devices — the same approach you would take when considering the acquisition of any diagnostic device.

“Choosing a measuring device should be based on the doctor’s confidence in using the device to obtain scientific information about macular pigment and not just for ‘marketing’ a nutritional approach,” he says.

Other MPOD-measurement tools — currently available only for research — may become more widely available, says Dr. Bernstein. Methods that could hold promise include more image-based ones, such as autofluorescence, Raman spectroscopy, and reflectometry.

Dr. Reed says clinicians also need to understand the limits of MPOD testing.

“MPOD testing is really measuring the levels of lutein and zeaxanthin in the macula, and we need to use caution when making the leap between macular pigment and AMD,” she says. “Certainly there is a strong association, but it may not be quite as linear as it would initially appear to be.”

Dr. Lange says he uses a blood test to see concentration in ug/l of lutein and zeaxanthin to help determine what dietary and supplement adjustments a patient needs.

Dr. Anshel raises another point in measuring these carotenoids: “Since lutein is sequestered in fat tissue, patient body mass index might be a factor in how much lutein is actually transferred to the eye.”

He adds that conversion of lutein to meso-zeaxanthin may also be an issue.

“Meso-zeaxanthin is converted from lutein by an enzyme, which needs to be present,” he says. “While we assume that it is, we have no way of being certain. This is relatively new science, so establishing a baseline might take more time.”

The bottom line: Be careful, and be educated

While the science is new and more research is needed, those interviewed for this article agree that with the proper education, optometrists can counsel patients on the relationship between nutrition and supplements, AMD and other eye diseases. An O.D.’s recommendation may also improve the overall care of the patient.

Says Dr. Lange: “I see it every day in clinical practice where we have made dietary and supplemental recommendations, and patients’ ocular health and overall health can improve dramatically.”
